Position Title

Coordinator Home Health Care - Administration (MSHC)

Position Overview

Coordinator, Home Health Care - Administration (McAuley Seton Home Care): Responsible for the development and implementation of home care services for LIFE participants, nursing home care and the provision of durable medical equipment. Supervises the delivery of all home caregiver staff to assure all care is being provided in compliance with all relevant policies, procedures and regulations and is based upon the individual plan of care.

REPORTS TO: LIFE Executive Director

SUPERVISORY RESPONSIBILITIES: Home Care RNs, Home health aides, Personal care workers

SKILLS and EDUCATIONAL REQUIREMENTS (minimum):
EDUCATION:
· Registered Nurse.
· Bachelor's degree in Nursing required.
· Current NYS RN License.

EXPERIENCE:
· Three (3) to five (5) years of related experience in home care, long term care, or PACE.
· Two (2) years previous management required particularly in community health or home care.
· One (1) year of experience required working with a frail or elderly population and must demonstrate their competence to perform their functions.

· Effective leadership skills necessary to develop a collaborative working relationship within a team composed of various health care professionals.
· Strong communication skills that assist team members to understand and articulate program goals, mission, and vision.

· Ability to provide an innovative approach to planning and problem solving.

· Interpersonal Effectiveness.
